Prosthetic aortic valve: elevated velocity or gradient

Advanced
Version 2026.10.1

Valve context, flow state, sampling check, jet contour and timing, DVI, indexed EOA or EOA versus reference, and comparison with the designated baseline. It never grades structural valve deterioration from a gradient change and never separates thrombus from pannus by Doppler.

Applicability
  • Surgical or transcatheter aortic prosthesis with an elevated CW peak velocity (>3 m/s). Mitral, tricuspid and pulmonary prostheses are not covered here.
  • Significant stenosis should meet at least one flow-dependent and one flow-independent parameter; other causes of an elevated gradient must be excluded.
  • This pathway does not diagnose structural valve deterioration from a gradient or velocity change, and it does not separate thrombus from pannus by Doppler: structural change must be seen on imaging, and etiology of obstruction is established with contrast CT and/or TEE.
  1. Table 5, SAVR versus TAVI rows
Is the prosthesis surgical (SAVR) or transcatheter (TAVI / ViV)?
Prosthesis type (surgical or transcatheter) is needed: Table 5 uses absolute criteria for SAVR and change from baseline for TAVI.
